Students who are not currently enrolled in grade 8 at Albert F. Ford Middle School will need to have their residency verified by the Acushnet School Department prior to enrolling. Please begin your student(s) registration below.

Enrollment is streamlined by using our PowerSchool online enrollment forms.
Please read through the 4 steps below to familiarize yourself with our full registration process.
Step 1: Please call the school where you will register your student. Registration will be guided by the Principal’s Secretary or Registrar to avoid duplicate enrollments.

Step 2: Please provide the following required documents to the school where you are registering:
original birth certificate
school's academic records (transcript, IEP/504 records, attendance, discipline and test scores)
medical records: immunization record and copy of recent physical (within 1 year)
any legal documents such as custody agreements, court orders, etc.
evidence of residency, occupancy and identification: one document from each column listed below.
Step 3: You will then receive an email with instructions for creating a PowerSchool parent access account and instructions for linking your student to it.
Step 4: Complete all 8 enrollment forms as directed in the email. ALL 8 enrollment forms are required to be in submitted status.
* Enrollment forms are completed onlyonce.
* Annual forms must be completed each subsequent year.
Missing or incomplete documentation and forms will result in delayed registration.

copy of lease agreement and record of recent rental payment
landlord residency affidavit and record of recent rental payment

Recent bill dated within the past 60 days showing a Marion, Mattapoisett or Rochester address:
